Let’s get you traveling!

In order to confirm your trip, we will need authorization to charge your credit card on your behalf. You should never see any charges to your credit card that are not related directly to the merchant you are traveling with. If you have any questions, please let us know. We also need you to review all the terms below. These are very important guidelines to make sure you have no hiccups during your travel!

I authorize Seek the Destination Vacations to use this information to directly make payments to travel suppliers for my approved vacation plans. I certify that I am the authorized user of this credit card. I will not dispute payment with the credit card company, as long as the transaction corresponds with the information on this form. I have reviewed my trip decisions and details and confirmed all information is correct and I understand changes may result in additional fees. I acknowledge that I am purchasing this trip from a travel agent who has helped me plan and book my travels and not directly from the travel supplier.

Cancelation Terms:

I acknowledge that have received, read, and agree to any cancelation terms.

Travel Documents:

I understand that is is solely my responsibility to make sure that I have up to date travel documents including passports, birth certificates, visas, and vaccination records if required. I understand that I am responsible if any additional fees or other consequences arise for not having these documents. I am aware that any travel requiring a valid passport also requires passport validity to have at least 6 full months after my planned return date.

Travel Insurance:

I am aware that I am responsible for my own travel insurance, whether purchased through a partner of Seek the Destination Vacations, from the travel supplier directly, or purchased on my own. I understand that if i waive travel insurance, I will be responsible for any and all charges or lack of refund if I have to cancel or I am otherwise unable to make my travel.

Airport Arrival Time:

I understand that I am responsible for arriving to the airport at least 2 hours before domestic flight departure time and at least 3 hours for international flight departure times. I understand late check in may result in denial of boarding and/or additional fees.

Authorization of this Agreement:

I understand that by typing my name below and submitting this form constitutes the execution of this agreement in exactly the same manner as if I had signed, by hand, a paper version of this document.
